about the difference between the prices in Thailand
and Germany:

For a better understanding we have added some pictures
as well. All these pictures were taken in our clinic
during treatment with a simple digital camera. Out
of consideration for our patients the treatment has
been stopped only for a few seconds. We hope to have
your understanding for the unsatisfying quality of
the pictures.

For a better comparison we have also listed the German
prices. In Southeast Europe the prices are slightly
below the German level, in Switzerland, Austria, England,
Sweden and Norway dental prosthesis normally cost
more. In the USA prices are much higher.
CROWNS:
If teeth have large fillings or insufficient crowns,
I suggest ceramic crowns.

This patient got crowns a couple of years ago.
It is obvious, that the dental technician was not
sufficiently experienced with ceramic, as the crowns
look very artificial and not aesthetic.
The use of low quality products and a suboptimal drill
technique of the dentist had resulted in a gingival
shrinkage, so the dark edge of the crowns became visible.

After a periodontal treatment and a new preparation
of the teeth with special instruments, we could fix
perfectly aesthetical crowns. Due to my further education
in periodontology and gingival surgery, I was able
to repair the damage and thanks to the long job experience
with ceramics of my dental technician, we could produce
perfectly aesthetical perfect and natural looking
teeth.

BRIDGES:
If there are one or more teeth missing causing gaps
between your teeth, I suggest ceramic bridges.

This patient had large gaps in the upper and lower
jaw. Because of the high prices in Europe, he had
a simple clasp denture. As you can see, the clasps
have already damaged a few of his teeth, especially
in the cervical area. The patient was afraid of further
tooth loss and asked for bridges..
In the Euro Dental Clinic
he received perfectly aesthetical bridges made of

TELESCOPE
DENTURE :
In case of large gaps or missing teeth in the lateral
area of the jaw, a so called free end situation, I
suggest telescope dentures.

First primary crowns are fixed on the remaining teeth.
The secondary crowns are fixed in the denture.

When the telescope denture is put on the primary crowns,
they slide into the secondary crowns,
which are fixed in the denture and fit with high precision,
which results in a very stabile retention,
much better than any clasp denture.
We call it a telescope denture because primary and
secondary crowns slide in each other like the parts
of a telescope. The big advantage is, that the patient
will never need a new denture again. In case of tooth
loss, we simply put a little amount of acrylic resin
in the corresponding secondary crown. Other denture
systems require a complete new construction in such
a case..
Because of a few unpleasant and painful experiences,
this patient had not seen a dentist for years. As
he told me later, he lived very lonely and had almost
no social contacts, because of his ugly dental situation.
As his toothache became more and more unbearable,
he had no other choice than getting a dental treatment.
He told me, that he only wanted a pain-relief. After
the treatment, I suggested a second consultation.
Together with my dental technician, we explained the
different possibilities of modern dentistry and suggested
a telescope denture. I had to promise, that the treatment
will be almost painless and finally he agreed.
About two weeks later he had a bright smile again.
As he told me later, his life has completely changed
and he has become a person full of the joys of life.

IMPLANTS:
Implantology is one of the most
exciting developments of modern dentistry. For the
first time in history it is possible to replace almost
any lost tooth with an artificial root.
On the Implant market there are leading companies
with decades of experience, high class products and
branches all around the world; but unfortunately also
producers of cheap low quality implants.
Implants work like a building kit. For example: If
you have an implant with a single crown on it and
a change of your dental situation should occur afterwards
(tooth loss) the same implant can be used even years
later to place a bridge or even a telescope denture
on it after changing one or two parts of it.
Low quality implants pose two kinds of possible problems:
Sometimes the bone does not heal because of their
low quality, which may even cause life endangering
infections. Or there are no “spare parts”
available, because the company does not exist any
longer, which also may lead to massive problems.
I have provided implant surgery for
more than 10 years. After a long time of experience
and lots of further education, I was awarded with
the title “Geprüfter Experte der Implantologie”
(examined expert of implantology) by the DGOI (German
association for oral implantology) and with the title
“Diplomate of Implantology” by the ICOI
(International Congress of Oral Implantologists).
I use exclusively high quality implants from leading
producers.

Implants are made of titanium. They are inserted in
the bone and after a healing period of 6 up to 12
weeks a prosthodontic application (crowns, bridges,
dentures) is placed on.
The advantages are that:
1.
In case of a single gap
(only one tooth missing) there is no need to drill
into the teeth next to the gap.
2.
In case of a free end
situation (when the teeth on the left or right posterior
part are missing) we can
avoid
a partial denture and
3.
In case of edentia (all
teeth are missing), we can extremely improve the retention
of a full denture or
even avoid one.
For a better understanding, I have
asked a few of my patients to describe their experiences
with their own words:

Andrea B., 68 years:
I had full dentures in both jaws for a long time without
any problems. But in recent years retention became
worse and they had to be underlined many times, because
my jaw bone was not good anymore. The upper denture
still has some retention because of the suction on
my palate but the lower one had no retention at all.
Even the adherent paste did not help anymore. Before
eating, I had to cut my food into small pieces. My
dentist suggested implants but my health insurance
didn’t want to pay for it, though I could not
eat well anymore and had to see my doctor many times
because of stomach problems. Because of my small pension
I couldn’t afford it. Due to mild weather and
low costs, I spend each winter in Thailand and so
I decided to get the treatment. I saved more than
50% of the costs, although I even got German implants.
The surgery was absolutely painless. A couple of weeks
later, I was able to chew again, even a carrot was
no problem. My lower denture fits tight and does not
move at all. Today I can’t imagine anymore how
I could live without it.

Herr Horst M., 62 years:
I had periodontal problems and my lower cheek teeth
became loose and had to be extracted. And then the
problems started. I could not chew anymore. I even
asked for implants, because I had read about them,
but my dentist didn’t do implant surgery. He
told me, that implants were very painful and not good
and would be lost soon. So I got a partial denture,
which was fixed on my front teeth with clasps. It
was horrible. I was choking all the time and there
was always some food sticking under it. Finally I
didn’t wear it at all.
Dr. Ramin Yachkaschi inserted two implants in my lower
jaw, one in each side. The surgery took only one hour
and after one month I got two bridges, which were
fixed permanently on one implant and two teeth on
each side of my lower jaw. I can’t feel any
difference to the teeth I had before, they feel great.

Herr Hans F., 48 years
I always had good teeth; I am almost 50 and have just
a few fillings and no crowns. I was very proud about
that and always took good care of my teeth. Then it
happened: One of these pick up taxis ignored the right
of way and I fell from my motor bike directly on the
street. I wore one of these helmets, which are open
in the front area and so my face was not protected
and I lost my upper right lateral incisor tooth. Some
dental technician suggested a bridge but I didn’t
agree because two of my healthy teeth had to be drilled
in this case. Dr. Ramin Yachkaschi told me everything

the implant was inserted and six weeks later a crown

COMPOSITE
FILLINGS:
Composite filling is a high quality
as well as very gentle filling therapy, which is more
beneficial than amalgam or simple white resin fillings.
Composites can be used in the front teeth area as
well as in the molar area. They are made in the same
colour of your teeth and are very durable.
Often we can avoid
much more expensive crowns.

This patient had simple resin
fillings in the cervical area of some of her teeth,
which did not fit to her tooth colour. The result
was more caries around the fillings after just a few
years. After we filled them with composites, the massive
defects almost disappeared.

Here you can compare an amalgam filling with
a composite filling. While the amalgam filling shows
cracks between the filling and the tooth, the border
between the composite and the tooth is almost invisible.

This customer was discontented with his upper
front teeth, because they were worn and had caries
in the cervical area. A dental technician had suggested
him to get all his teeth crowned, which made him a
little suspicious. Hence he came to the Euro Dental
Clinic for a second opinion. As you can see, we filled
three teeth on his upper right side with composites
first. Therefore we had to drill them just for a few
seconds which required just a minimal loss of tooth
material. In case of crowns we would have had to drill
much deeper and the customer would have to pay a much
higher price. After the patient was convinced of the
high quality and aesthetics of composites, he agreed
to get his other teeth filled with composites as well.

CROWN
PRODUCTION :
On these pages we would like to describe the production
process of crowns. As you will see, there are various
ways of producing crowns or any kind of prosthetics,
depending on whether you want a cheap product of low
quality or a perfect one, manufactured according to
modern standards of science, highest level of aesthetics,
biocompatibility, precision and durability.
Unfortunately a patient is able to judge the aesthetics
of dental work but not the quality of material and
precision, because even low quality and imprecise
crowns can last a few months or even years, but then
it is too late for any sort of recourse.
After studying these pages, you will be able to judge
your dental treatment and the production process of
your crowns.
Your health is important!
1. Consultation:

This patient had a simple veneer metal crown made
of cheap alloy and covered with simple resin on the
outside surface on her upper right incisor tooth.
She was unhappy with the unnatural colour and the
fact that the metal rim of the crown was visible in
the cervical area.
Because of the large filling in the second incisor,
she asked for two new crowns.
2. Examination:
The insertion of crowns puts
an extraordinary stress on teeth. They are drilled,
a lot of impressions are taken and they have to tolerate
the fixing of temporary and final crowns. And if they
are supposed to be abutment teeth for some bridge
or other kind of prosthetics the stress level is even
higher.
Therefore it is extremely important to clarify in
advance, if the affected teeth will survive this procedure
and how long they will last afterwards (prognosis)
and if there are any other inhibiting factors infections,
periodontitis, cysts, etc.)
This is managed by an intraoral inspection and a palpatoric
examination with special instruments and by x-ray
pictures, which facilitate an examination of the teeth
under cariologic, periodontologic, endodontologic
and prosthetic aspects.
Under no circumstances should the consultation or
even examination be done by an incompetent person.
A dental technician or dental assistant may be able
to decide, that for example a tooth with an insufficient
crown or filling needs a new crown. However, they
neither posess the required knowledge based on university
studies nor sufficient experience to be qualified
for a correct examination of teeth, an exact judgement
of x-ray pictures and therefore a reliable prognosis.
3. Preparation:

First we determined the colour and took some impressions
for the production of the temporary crowns.

After the injection we removed the old crown. We found
a large carious defect and had to put a titanium pin
in the tooth for the following filling.

After the drilling of the two teeth, we took a high
precision impression and sent it to our dental lab
together with several other impressions and bite registrations.

At least we inserted some fluid resin into the first
impression and placed it back into the patient’s
mouth, where it hardened after a few minutes. After
polishing we fixed temporary crowns on the teeth.
4. Lab
Production:

The technician uses dental gypsum for the production
of casts.

After preparation of the teeth in the cast, the technician
produced small caps made of wax, which were transformed
into ceramic or high gold alloy.

Afterwards many different layers of ceramic were put
on the cap and baked in the ceramic furnace.

Because of the various hues in the different areas
of the crown and the use of transparent ceramics in
the incisal area, you simply can’t tell these
crowns from natural teeth.

Many steps are done under a microscope, to accomplish
high precision and therefore long durability, because
only a perfect fitting between crown and tooth guarantees,
that no bacteria can move under the crown, which would
sooner or later lead to loss of the crown or even
the whole tooth.
Unnecessary to mention that we use only high class
materials for the production of this crown for an
optimal biocompatibility
5. Fixing:

Finally the crowns were fixed with a special adhesive.
The result speaks for itself.
Functional analysis:
The human jaw is a multifunctional joint, which means
it is capable of movements in all directions (lateral,
forward and backwards) and not only for opening and
closing movements. Normally there is harmony between
the joints and surfaces of lower and upper teeth during
movements of the upper and lower jaw under tooth contact,
for example chewing.
Any disturbance of this harmony, for example caused
by teeth malposition or misfit dental prosthetics,
can sooner or later lead to a temporomandibular damage
with serious consequences like strong head and neck
pain or even balance impairment or tinnitus.
Therefore it is extremely important, that dental prosthetics
harmonizes with the jaw in any possible movements
and not only opening and closing movements. This is
only possible, if the prosthetics is produced under
functional analytical aspects.
Functional analysis
is a registration of all individual parameters of
possible jaw movements of the patient, which differ
from person to person.

Afterwards these parameters are transferred into an
adjustable articulator,
which can simulate all jaw movements and in which
the prosthetics will be produced, exactly fitting
to the individual jaw situation of the patient.
The simple crown:
This crown is produced of cheap alloy with almost
no gold or platinum inside and covered with only one
layer of resin or ceramic on the outside surface.
After the production of the cast, the technician manufactures
a crown of wax, which he transfers into a metal crown
made of low gold alloy.

Afterwards one single layer of resin or ceramic is
put on the outside surface. The crown is produced
in a simple articulator, which is only capable of
opening and closing movements but not able to simulate
the individual movements of the patient. The lack
of the functional analysis will sooner or later lead
to temporomandibular damages.

At first glance the result is quite acceptable but
can’t be compared to the full ceramic crowns.
The use of low quality materials may lead to discoloration
and to irritations of the gum and sense of taste.
Besides these crowns are not manufactured under the
same precision techniques like the high quality ceramic
crowns, which reduces the durability and may require
a new treatment after a few years.

PROPHYLAXIS:
99% of all adults suffer from caries or periodontitis.
Therefore in Germany every year more than 70 Million
teeth are filled, 13 Million are crowned and 15 Million
are extracted, because they couldn’t be saved.
It is amazing how easily we can take, that a tooth
is filled or even extracted. With no other part of
our body we would accept, that it is rotten and replaced
with plastic or metal for the most part, before reaching
half of one’s life expectancy.
Caries and periodontitis are infectious
diseases caused by bacteria:
The oral cavity is populated with countless bacteria,
which stick on the surface of the teeth and produce
the dental plaque. In some areas the plaque hardens
and cannot be removed by simple tooth brushing.
The excretes of these bacteria are acids and toxins,
which attack the teeth, the gum and the jaw bone and
result in caries (tooth decay), gum bleeding and periodontitis
(loose teeth).
Today we know, that tooth brushing,
twice a day, is not enough to avoid caries and periodontitis.
Clinical experience of the last years has proven,
that patients who get an individual prophylaxis treatment
regularly can keep their teeth healthy up to an old
age.
Individual prophylaxis is an individually adjusted
care system to avoid caries and periodontitis.
If caries and periodontitis are caused by known bacteria,
why do we need an individual care system?
Because the reasons why teeth are infested with bacteria
differ from patient to patient and depend on genetic
handicaps, dental malposition, insufficient fillings
or prosthesis, composition of saliva, ineffective
cleaning technique, nutrition and even sleeping habits.
The objective of the individual prophylaxis
is to determine individual risk of the patient to
suffer from caries and periodontitis and to eradicate
them.
First we have a talk with the patient
and analyze his nutrition and dental hygiene habits.
Later we examine the mouth cavity and teach the optimal
dental hygiene technique.
The most important part of the individual
prophylaxis is the PTC, the professional tooth cleaning.
As already mentioned, tooth brushing twice a day and
tartar scaling twice a year are not sufficient to
avoid caries and periodontitis, because there are
various kinds of plaque, caused by coffee, tea, tobacco
and food colours, which stick on the surface of teeth
and support the growth of bacterial populations. These
kinds of plaque cannot be removed by normal tooth
brushing. Besides there are areas with a higher risk
of decay, because they cannot be reached by a tooth
brush, like crowding of teeth, interdental space or
insufficient fillings or crowns.

Please don't forget:
Caries
and periodontitis are not healable!
A hole in a tooth can only be filled by artificial
materials.
A loose tooth can only be extracted.
Certainly you have had some fillings,
crowns or even tooth extractions already.
To avoid this for the future, we will free your mouth
of bacteria, because:
Caries and periodontitis are infectious diseases caused
by bacteria.
A consistently carried out prophylaxis is the only
successful method to avoid caries and periodontitis
permanently.
Unnecessary to mention that the examination
and treatment should be done by experienced personnel
with modern equipment.s
The PTC starts with cleaning of tartar
with an ultrasonic hand-piece.
It is important that only equipment from leading companies
are used, which is serviced regularly. Low quality
instruments could destroy the dental enamel, if they
are not serviced and the vibration is not adjusted
by qualified personnel.
Later the rest of the tartar is removed by small scaling
curettes and files. These curettes must be sharpened
regularly, otherwise they could damage the teeth.
Afterwards the plaque and discolouring are removed
by an air flow hand-piece. The use of only high quality
equipment and materials is indispensable, otherwise
it may lead to damage of teeth or gum.
The finishing polish is done with special polishing
pastes.
